Medicare annual wellness visits and lifelong wellness

Staying on top of your health is even more important in your golden years, because when you take care of your health, it’s easier to stay connected to those you love. That’s why Medicare covers an Annual Wellness Visit (AWV) with your Adventist Health primary care provider. This free, one-on-one consultation doesn’t include a physical exam, but rather a discussion about your health needs and goals, plus a personalized prevention plan to help you meet them.

One-on-one discussion of your health and goals

Preventive care helps you manage your present and future health. The annual wellness visit is an opportunity to consider issues that may not be covered in a typical appointment. Your health care provider is a partner in your wellness, and the Annual Wellness Visit is their time each year to focus exclusively on you and to discuss:

  • A complete medication review to identify any potential adverse drug events
  • A review of your immunization schedule
  • Age-and gender-appropriate screenings
  • Medical, family, social, and physical activity history
  • Other Medicare-covered preventive services to make sure you get access to the care you need
  • Your current medical conditions, health status, risk factors, and health goals

What is a personalized prevention plan?

A personalized prevention plan is a great tool to help you stay on track to better health. After discussing your health status and goals, your provider will develop a plan for you in a booklet to take home that includes immunization schedules, lifestyle recommendations, and preventive screening schedules.

Annual exams are also important

A wellness visit should never replace your annual physical; it is intended to complement it. Your wellness visit is a conversation, whereas an annual physical includes a series of physical exams to give your doctor a better idea of your overall health.
